Intuitive Surgical (ISRG.US), the global leader in surgical robots, released its Q3 2026 earnings report on October 20 (U.S. Eastern Time). On the same day, JPMorgan published a research report, maintaining an "Overweight" rating on the stock.

According to the Zhihui Finance APP, global surgical robotics leader Intuitive Surgical (ISRG.US) released its Q3 2026 report on October 20 Eastern Time. On the same day, JPMorgan released a research report maintaining an "Overweight" rating for the stock, raising the target price to $475. The bank stated that in the fast-growing soft tissue robotics sector with low penetration rates, Intuitive Surgical is the best positioned company. Data shows the company’s revenue, profit, and gross margin all significantly exceeded market expectations. However, the key metric of U.S. surgical volume only met consensus expectations and did not reach the more optimistic forecasts of buy-side institutions, leading to post-market share price pressure after the report’s release.

Earnings greatly exceeded expectations, hardware installations performed strongly

The report shows total revenue for Q3 was $3.049 billion. Systems equipment revenue reached $769 million, a year-on-year increase of 30%; global da Vinci system installations reached 546 units, up 28% year-on-year. Installations of the next-generation dV5 reached 315 units, significantly higher than previous institutional forecasts. Both U.S. and international markets saw installation growth exceeding expectations, with outpatient surgery center (ASC) channel expansion delivering notable results.

Recurring revenue from consumables and services also performed strongly. Instrument & Accessories (I&A) revenue reached $1.8 billion, up 18% year-on-year; services revenue was $482 million, a 22% increase year-on-year. On the earnings side, adjusted EPS reached $3, $0.36 above market expectations, or a 14% increase; gross margin rose to 69.9%, and operating margin beat expectations by 230 basis points. This quarter, the company completed around $3 billion in share buybacks and raised its full-year gross margin guidance to 68.7%-69.3%. However, GAAP EPS was just $1.49, mainly due to a one-off pre-tax charge of $450 million relating to an antitrust settlement.

U.S. surgical volumes fell short of optimistic forecasts, overseas markets became the growth engine

Surgical volume is the core KPI determining Intuitive Surgical's valuation. Global da Vinci surgical volume grew 14% year-on-year, but U.S. domestic growth was just 10%, only in-line with market expectations and 1-2 percentage points below optimistic buy-side forecasts — this was the main reason for the stock price pressure. Due to a high base, U.S. device utilization rates fell quarter-on-quarter, and bariatric surgery volume saw a low double-digit decline.

In contrast, overseas markets performed strongly; da Vinci surgical volume overseas grew 20% year-on-year with strong momentum in Europe and Asia, India growing 42%, and Japan accelerating for two consecutive quarters thanks to improved reimbursement policies. China’s business growth was in line with the company’s overall average, but was constrained by a sluggish capital installation environment. Management narrowed full-year da Vinci surgical volume guidance to 14%-15%.

Price adjustments manageable; institutions suggest buying on pullbacks

The company introduced new pricing for longer instrument usage and, combined with price reductions in China, per-surgery consumables revenue is expected to face a 3-5% impact once all policy changes are implemented — lower than the 7% impact seen in 2020. The price adjustments will roll out in phases and their full effect will be gradually realized by 2027. Drawing on historical experience, management believes that price decreases may release latent demand elasticity, while cost reductions can offset gross margin pressure.

JPMorgan pointed out that 74% of the company’s share price volatility comes from fundamentals specific to the company itself. Despite the overall high quality of the report, the share price had already been elevated by previous third-party optimistic data and is expected to reasonably pull back by 3-5%. If the stock opens and drops further by about 7%, it could present a good buying opportunity. In the long term, soft tissue surgical robotics still has low penetration rates; with new indications and overseas market expansion, the company’s growth potential remains broad. Risks are concentrated in new product R&D, competitive pressures, and product safety incidents.

As of the close on October 20, Intuitive Surgical was priced at $438, with a market capitalization of about $158.8 billion and a 52-week share price range of $328.57–$603.88.
